Employer's Knowledge Affidavit

This form is required to outline the employer's knowledge of the prior permanent impairment and the employer's consideration of the impairment's permanency and hindrance to employment.  The form should be completed by the employer and signed by an individual who has the authorization to make personnel decisions or has significant input in recommending personnel actions.
